Patient: Am I eligible for an Opencare reward?

You are eligible for an Opencare gift card reward if all the below are true:

• You book a new patient exam and another dental service (like a cleaning). General consultations do not qualify for rewards.
• You, the patient, are at least 18 years old on the date of your appointment.
• You have not claimed an Opencare reward in the past 2 years
• Your appointment was booked through our online platform. Appointments scheduled by telephone, in person, or by any other means, do not qualify for rewards.
• Your appointment was booked using the name on your Opencare account. If one account was used to book 2 or more patients, only one patient will be able to claim a reward.
